Recipient Org: Description Children’s Hospice South West (CHSW) makes the most of short and precious lives by providing hospice care for life-limited children and support for their families. Regionally, there are over 1,100 life-limited children. Charlton Farm in Wraxall serves families in the West of England: Bristol, South Gloucestershire, North Somerset, Bath & NE Somerset. The hospice supports children from a few days old to 23 years of age. Families are supported from the point of diagnosis or referral, throughout the child’s life, into end of life care and through bereavement. Families are allocated 18 bed nights each year for respite. In addition, families also turn to us for emergency care and end of life care. Charlton Farm provides 6,000 bed nights each year to 800 life-limited children, parents and siblings.
