The impact of early life exposures on chronic respiratory illness in African children (360G-Wellcome-204755_Z_16_Z)

Childhood respiratory disease remains a leading cause of morbidity and mortality globally and particularly in low and middle-income countries (LMIC). Chronic respiratory illness (CRI) constitutes a large burden of disease both in LMIC countries and globally, with much CRI having its origins in very early life. Understanding the risks for early life lung disease and impact on lung growth is key to developing preventive and management strategies to reduce the burden of CRI. We aim to investigate the impact of early life exposures, particularly lower respiratory tract infections (LRTI), on development and progression of chronic respiratory illness in African children. We have followed 1000 infants enrolled in an African birth cohort, the Drakenstein Child Health study, from birth to 3 years, identifying antenatal and early life factors that altered normal lung function. LRTIs were identified as a factor, independent of baseline lung function, associated with lower lung function in infancy. We will build on this rich dataset to follow the cohort annually until 6 years. We hope to better understand the phenotypes, risk factors for, progression and modifiers of CRI in African children thereby identifying key preventive and management strategies to reduce CRI burden, particularly in LMIC settings.
