Scaling up the Well Centre’s primary care led, integrated youth hub model in Lambeth, Southwark, Lewisham and Croydon (SLaM boroughs) (3491)

£998,999
Recipient Organisation

Goal: Improve access to timely mental wellbeing support through the Well Centre's established integrated care model, reducing mental health disorder onset for Children and Young People (CYP) with emerging difficulties living in South London. • Challenges & beneficiaries: Current CYP mental health services cannot meet demand and exacerbate health inequalities. Services offered via CYP tailored primary care settings reduce barriers to care for marginalized CYP and enhance access to evidence based treatment. • Solution: Extend the reach of the Well Centre's established integrated care model beyond Lambeth, through a co-designed implementation and evaluation programme with multiple stakeholders in SLaM region.
