Recipient Org: Description St Catherine’s cares for patients and families across Chorley, Preston and South Ribble who are affected by life-shortening conditions like cancer, motor neurone disease and heart failure. Our specialised care is here to help people enjoy the best possible quality of life, to the end of life. We work with patients and their loved ones at the hospice and in their own homes to help them achieve what is important to them in the time they have. Our In-Patient Unit at St Catherine’s provides the most intensive round the clock care for patients and their loved ones. It can care for up to 19 patients at a time, with most in single rooms, although some are shared. Unlike a care or nursing home, we do not provide long-term care within the hospice since our services are very much in demand. Instead, we work to provide specialist interventions to help symptoms become more manageable, with more than half our patients then discharged home. Our medical and nursing teams, support team, physiotherapists, complementary therapists and volunteers work together to ensure that physical, psychological, spiritual and social needs are met during a stay with us. We recognise that the emotional support people need at this time is often just as important as the physical care which is needed. St Catherine’s also cares for up to 200 people at a time through our Clinical Nurse Specialist (CNS) team, who visit people at home across Chorley, Preston and South Ribble, including prisons, nursing homes and care homes. They provide assessment, treatment and support, and work together with GPs, district nurses and other health and social care professionals to ensure care is co-ordinated. They are available seven days a week, offering expert knowledge of complex symptom and pain management alongside psychological support. Their aim is to help patients achieve the best possible quality of life in the places they call home. The support they offer at the end of life is also invaluable to people who would prefer to die in their usual place of residence, rather than in a hospital or hospice. The community team also includes the Hospice Support Assistants who work very closely with the Clinical Nurse Specialists. Although they are not clinically trained, through working closely with the CNS team they have developed knowledge and skills to help patients manage certain symptoms. They have skills to help patients and their carers manage issues such as fatigue, anxiety and breathlessness. The practical advice and emotional support they are able to offer in these areas can have a significant impact on improving people’s quality of life. Our lymphoedema service is based in the Woodside Clinic within the grounds of St Catherine’s Park. The award-winning team treats cancer and non-cancer related lymphoedema – a condition which affects our lymphatic system, the process by which we filter and drain waste fluid from our bodies. The team at St Catherine’s offers treatments to ease discomfort, improve quality of life and boost independence by helping you to understand and cope with your treatment, and advising you how to self-manage your oedema with techniques such as exercises and massage. We also have a dedicated Support Team which is here to help carers and loved ones in a range of practical and emotional ways. They also support carers in the community who don’t have to be connected with the hospice. The team is made up three qualified social workers who work with patients and loved ones on our in-patient unit and in the community. The team hosts a monthly carers’ drop-in support session and manages our volunteer-led Befriending Service which provides a lifeline to carers, allowing them to take a much-needed break. The care of St Catherine’s doesn’t stop when a loved one has died – we’re here to support loved ones through bereavement, and to provide opportunities for our community to remember and reflect together. Bereavement support is offered through all of our community and hospice-based teams and involves various different people including doctors, nurses, social workers, volunteer chaplains and volunteer counsellors. The Bereavement Support Team therefore provides a number of different ways to help, working either on a one-to-one basis or in a group – whichever people feel best meets their needs.
