Recipient Org: Description CLARE is a community development social work project with the mission to address social isolation, improve health & well-being and promote independence by empowering individuals to shape and manage their care and support, drawing on skills within local communities. We do this by taking referrals of vulnerable older people from Belfast health trust social work teams and GP practices in North Belfast ( there are some self referrals) our registered trained and experienced Social Workers (1.5FTE) visit the person and co produce a living plan to enable them to have a better quality of life - we then work with both volunteers in the community, recruited and supported by our Volunteer Coordinator, and with our Community Wellbeing Officer to support the person make the living plan a reality. The person is assisted with practical tasks like shopping, socialising, navigating the statutory system and attending GP and other appointments , volunteer befriending and critically we also connect the person in to support and activities being offered by other VCSE and statutory bodies locally following an asset based community development approach. We worked with more than 100 referrals last year - mostly people with serious vulnerabilities. We use a bespoke client management system called Clarity to accept referrals and track all our work.
