| Recipient Org: Description | CLARE is a charitable Community Interest Company working through community development social work with the mission to address social isolation, improve health & well-being and promote independence by empowering older vulnerable individuals (65+) to shape and manage their care and support, drawing on skills within local communities. We do this by taking referrals of vulnerable older people from Belfast Health Trust social work teams and GP practices in North Belfast ( there are also some self referrals) our registered trained and experienced Social Workers, 2 staff (1.5FTE) visit the person, develop a meaningful personal relationship and co produce a living plan to enable them to have a better quality of life - we then work with both volunteers in the community, recruited and supported by our Volunteer Coordinator, and with our Community Health and Wellbeing Officer to support each individual make the living plan a reality. Each person is assisted as appropriate with practical tasks like shopping, socialising, navigating the statutory system and attending GP and other appointments , volunteer befriending and critically we also connect the person in to support and activities being offered by other VCSE and statutory bodies locally following an asset based community development approach. We worked with more than 100 referrals last year - mostly people with serious vulnerabilities and from across and within all communities in north Belfast. We use a bespoke client management system called Clarity to accept referrals and track all our work and track the progress of individuals using the Outcomes Star for Older people. |
