Wellbeing Support Services - Wellbeing Toolkits (360G-CheshireCF-A570315)
The Haven provides timely and responsive programmes of support to help families across North and South Lanarkshire, affected by life limiting illness (e.g. cancer, Parkinson's, M.S, dementia and those in the covid-19 high risk and shielding categories) to cope with the physical, emotional and practical aspects of illness and/or caring responsibilities and to enable them to face the future positively and with hope. The Wellbeing Toolkits project will provide vulnerable clients with therapeutic resources bespoke to their individual needs to enhance Haven programmes of care and help them progress towards personal outcomes. Funding will support the delivery of this project by contributing towards the core staff salary costs of Project Co-ordinator (overall responsibility for project development and delivery, monitoring, evaluation and impact measurement), Emotional Support Practitioners (collaboratively identify client needs; provide dedicated and ongoing emotional support; support clients to make use of toolkit resources; identifying emerging client needs to steer further project development) and Admin Co-ordinator (supporting administrative processes – ordering of resources and posting toolkits). This project aims: to improve the emotional and mental wellbeing of Haven clients; increase self-management skills; build resilience; reduce stress and anxiety; and reduce the impact of loneliness and isolation.

Recipient Org: Description | The Haven aims to enhance the quality of life and emotional wellbeing of individuals affected by life limiting illness (cancer, Parkinson’s, dementia, MS and covid-19) across North and South Lanarkshire, their carers and wider family network. The Haven provides programmes of support designed to help individuals cope with the physical, emotional and practical aspects of their illness and to enable them to face the future positively and with hope. Support is offered at all stages of illness: at diagnosis; during and after treatment; at end of life, including pre/post bereavement support for families. We also provide support to children and young people to feel resilient and strong, enabling them to understand their own emotions and to be confident enough to share the natural feelings of loss when someone they care about is seriously ill or has died. The Haven operates three drop in centres, located in Blantyre, Wishaw and Forth and services are delivered by an experienced multidisciplinary team. Clients are supported via a range of 1:1 specialist support, peer and group based support, empowering and promoting self-management skill development. Due to the restrictions in place as a result of the covid-19 pandemic we have had to temporarily close our three Haven drop in centres. In response to this we proactively put measures in place to ensure that our vulnerable clients could continue to immediately access responsive support services with no break in support to clients: Haven staff immediately began working from home and we established our new Wellbeing Support Services to deliver the same high quality 1:1 emotional support services via telephone support calls and video support sessions. We have rapidly been developing these new Wellbeing Support Services in line with client needs with recent innovations including: MorphFit Gentle Movement online group exercise programme; Healthy Lymphatics (information and support for individuals at risk of or affected by lymphoedema); Lifting Your Spirits relaxation course. All our new Wellbeing Support Services are delivered with the same underlying principles and ethos as our previous centre based services. Our Emotional Support Practitioners actively support clients to navigate and manage their illness and/or caring roles within this new and challenging environment. All Haven support is specifically tailored to each individual and collaboratively developed by the client and their dedicated Haven Practitioner, using our ‘What’s Important Now’ (WIN) framework – a strengths and assets based approach - which helps our clients identify their own goals and empowers and supports them to take positive steps to achieve these goals. 1:1 Haven support sessions, signposting/referrals and links to wider statutory and community resources all aim to increase emotional and mental wellbeing, increase self-management skills, build resilience and, very importantly, reduce the impact of loneliness and isolation, which is an added challenge for our client group at this time. The most recent development of these new Wellbeing Support Services has been to introduce bespoke Wellbeing Toolkits which provide clients with resources tailored to their individual needs and interests to support their emotional, physical and mental wellbeing. |
