Day Hospice at Home (360G-CheshireCF-A572909)
Whilst our Day Hospice remains closed due to the Covid-19 pandemic we continue to support our terminally ill patients who are living in their own homes. We plan to provide on and offline activities to prevent loneliness and social isolation and improve physical and mental wellbeing over the winter months. This includes exercise classes to help people keep active indoors, creative activity packs to provide immersive enjoyable pass times to prevent boredom and telephone calls to provide reassurance and company. It is vital that we reach, engage with and support our vulnerable patients at a time when Coronavirus is increasing anxiety and posing a significant risk to the health of those with weakened immune systems, leaving many feeling housebound and alone. With our enhanced support patients can positively face the winter with increased resilience.

Recipient Org: Description | St Gemma’s Hospice has been providing the best possible care and support to the people of Leeds and beyond, free of charge for 40 years. We touch the lives of over 3,000 local people each year, caring for patients with cancer and other life-limiting illnesses, whilst also supporting their family and friends with services including: • In-Patient Unit providing 24-hour care promoting comfort, dignity and choice • Nurse-led beds for patients who choose not to continue active treatment • Vibrant Day Hospice facilities enhancing quality of life at the end-of-life • Community Nurse Specialists providing support in patients’ own homes • Therapy Support including physiotherapy, occupational and complementary therapies • Family Support comprising social workers, bereavement support workers, spiritual care team, support for carers and a specialist city-wide Young People’s Service As the first Hospice in the world to achieve University Teaching Hospice status our Academic Unit of Palliative Care (AUPC) is recognised for its extensive evidence-based research, learning and teaching in collaboration with the University of Leeds. We are now the largest Hospice in Yorkshire and work in partnership with Wheatfield’s Sue Ryder Hospice to support people from across Leeds. Wheatfield’s traditionally care for patients living in the west of the city, and St Gemma’s the east, although this agreement has the flexibility to respond to changes in demand and patient preference. Our Future: Our services have become all the more vital in recent months due to the Coronavirus pandemic. We have adapted our services to meet increased demand, strict Government guidelines and have cared for patients who have tested positive for Covid-19. We continue to strive to care for our community with flexibility during these uncertain timeS with enhanced services including bereavement support and help to arrange a funeral, specialist end-of-life care training for health professionals including in a care home setting and video consultations with patients during a time of reduced footfall in the hospice. Our vision, to meet the needs of people living with a terminal illness and those close to them with care, compassion and skill, and our purpose, to acknowledge the value of life and the importance of dignity in death, will not waver during this challenging time. |
