The Accommodation Gateway already works across the city and has established partnerships with LYPFT and other housing providers in the city. Therefore, the infrastructure is already in place to facilitate referrals. The Service Manager will oversee the project and promote the Winter Warmer Packs across the partnership, through provider meetings, social media and existing newsletters. Pack lists will be created with pick and mix options for clients to promote choice from within a budget, workers can take these to clients and help them to create their individualised packs. Outcomes measures & feedback structures will be set up to measure the impact & learning of the project. On-going measures of the wider impact for clients based around maintained tenancies and reduced re admissions to hospitals will be monitored via the Care Programme Approach & ReQoL quality of life outcome tool held within LYPFT statutory services.
