Cyrenians' Digital Inclusion Health and Care project will expand current digital inclusion support for people living with moderate to severe mental health conditions in Edinburgh. The project aims to help people to access a range of digital health and care tools, resources, and services. The project will increase awareness and use of self-management tools, improve digital access to health and care benefits, and enhance referral routes through Community Link Workers and GP practices. The project will support 20 people.

Grant Programme: Description | This fund is by invite only The Progress Fund offers continuation funding of up to £25k for 6 months to support organisations currently funded by the Digital Pioneers Mental Health or Housing Fund. • The fund will expand on the work undertaken during the Digital Pioneers Fund, with a greater focus on enabling people to connect with health and care services using digital.• Organisations will be asked to consider their current and prospective referral routes or pathways, from and to, health and social care services and will be supported to strengthen and develop these pathways. Therefore, we do not anticipate high numbers of people to be supported in this funding period.• Organisations will explore opportunities to improve individuals’ access to health and care supports using digital by incorporating this into current digital inclusion delivery and working in partnership with health and social care services where possible.• Organisations will be asked to complete Impact Surveys with individuals contributing to an evidence base for how digital inclusion support impacts their connection to health and care services.• Funding will support organisations working with people most at risk of digital exclusion, taking an intersectional approach supporting people with protected characteristics or experiencing multiple deprivations.• Projects will be supported for 6 months.• Projects will be required to participate in a Community of Learning which will explore sustainability and best practice models for Digital inclusion delivered in the areas of Mental Health or Housing. Fund OutcomesSuccessful applicants will be expected to meet the following fund outcomes:• People will have increased awareness of digital options for self-care, self-management approaches and community supports and connections that can benefit their health and care needs.• People will have improved digital skills and confidence, enabling them to access and connect with digital services and resources appropriate to their health and care needs.• Mental Health/Housing organisations will be better equipped to connect with health and social care services through digital inclusion support (for example this may include but is not limited to: understanding referral routes and pathways into digital health and care services/supports; understanding digital inclusion models specific to the health or care service and the people supported; building partnerships with health and care services to support awareness and prioritisation of digital inclusion in service/organisational policy). • Organisations will contribute learning to better understand the value of digital inclusion in supporting access to mental health/housing/health and social care services/supports and the impacts for individuals and services/organisations.
