Recipient Org: Description When somebody is given a diagnosis of a life-limiting illness, everything changes instantly for them and their loved ones, and there is nothing more important than making every remaining day count. At St Margaret’s Hospice, it is our mission to provide high-quality responsive care for these families. We’ve been at the heart of Somerset’s community for over 40 years, providing a vast range of services in community settings and from our Hospice Hubs in Taunton and Yeovil. Last year, we supported more than 5,000 people through our medical expertise, person-centred nursing, and specialist therapy services. Our main services are our: • In-patient Unit in Taunton, which provides a comfortable environment for those with complex care needs, where patients receive extra support, symptom control, therapy and pain management and, for some, end-of-life care from specialist teams • Sunflower Centre Day Services, where we welcome those who are well enough to live at home but who need to access our onsite holistic care • Community Teams – providing specialist assessment and intervention in the familiar environment of a patient’s own home through nursing, physiotherapy, occupational therapy, social work, and doctors (260+ patients at any given time) • 24-hour referral line and adviceline, receiving an average of 786 calls a month from patients/families who need advice from a palliative care nurse, and health professionals making referrals or seeking our expertise • Outpatient Services – offering a range of services including lymphoedema treatment, physiotherapy and complementary therapies (e.g. reflexology) • Supportive Care Service – supporting patients, families and carers who will lose/have lost a loved one to a life-limiting illness. This includes helping children and young people cope with the loss of a parent. • Spiritual Care Team, who provide direct care to patients, families and carers, working closely alongside clinical teams to ensure that we are caring for patients’ spiritual wishes alongside their physical and emotional needs We’re incredibly proud of our achievements in the last year, including supporting more people than ever before. The best thing is that St Margaret’s has continued offering 24/7 care and support through the entire pandemic, providing a lifeline for families when other services across the community were unable to. We have continued to meet the needs of patients, families and carers through a combination of face-to-face and remote support. The people we support have told us they value in-person provision so, now that restrictions have eased, we’ve recently been able to increase the access to our Sunflower Centres (SFCs), which were significantly affected by the pandemic. We continue to risk assess the delivery of services to protect the safety of our vulnerable patients.
