Randomisation within the database: a feasibility trial evaluating the comparative effects of statins in a research network of general practices (RETRO-PRO). (360G-Wellcome-086136_Z_08_Z)
The first part of this proposal will establish the best scientific design for randomised trials within the database and the best approach for obtaining patient consent. For example, it will be evaluated whether patient consent is always required for control patients if medical practice does not change. The second part of the proposal will recruit a total of 700 general practices interested in participating in these trials and update the software used in these practices in order to collect stud y-specific data. The third part will consist of a feasibility trial, randomising 300 statin starters to simvastatin or atorvastatin. Pharmacogenetic material will be collected at baseline and lipids at baseline and week 6. Patients will then be followed passively and life-time for cardiovascular outcomes linking the study patients to data collected in hospital episode statistics and a national disease registry of myocardial infarction. This study will provide evidence whether randomised tri als within the database can be used for studying long-term effects on major clinical outcomes in a representative general practice setting. If successful, this research network will be accessible to academic groups and allow the evaluation of other clinical questions.
