Moving On Project (360G-tnlcomfund-0010327187)

This group will use the funding to provide intensive tailored support to young people with profound and multiple learning disabilities (PMLD), and their families, during the transition from secondary education and child services to adult service provision. The support will focus on working with young people, their family carers and universities/colleges to meet the aspirations of the young person. This will include the development of digital communication passports for each young person.
