Compassionate Communities: Living Right Up to the End (360G-tnlcomfund-0010355427)

This group will expand and develop their 'Compassionate Communities' project, building resilient communities to provide appropriate locally-based support to people living with a long-term condition , carers and the recently bereaved. Activities will be aimed at reducing isolation and improving the quality of life for people affected by long-term conditions. Over the next three years the group will work with approximately 2,500 people, supported by 30 volunteers.
