The Outreach Project (360G-tnlcomfund-0020102686)
Sebastian's Action Trust (SAT) will use funding to continue its Outreach Project, which provides a team of family support workers to offer practical, social and emotional support for seriously-ill children and young people and their families. The project will continue to ensure that families reduce their isolation through access to a peer support network, befriending support, and improved engagement in the local community.
