This is funding so that Long term Conditions Alliance Scotland can create full business proposal to deliver a Trust aimed at supporting a process of transformation at critical stages in the lives of vulnerable people - young people leaving care and older people who have had a dignosis of dementia and their carers. LTCAS intend to deliver the Trust in partnership with two other organisations. Their approach will both build personal capacity and resilience and support service providers as agents of change.
