Healthy Transitions - framework for disadvantaged young people aged 16-25 (360G-tnlcomfund-0030136583)

The aim of this three-year nationwide initiative is to improve the well-being of over 8,500 young homeless people, aged 16 to 25 housed by 120 organisations (or foyers) across England. Seven projects will be split across three years and delivered at 40 foyers per year. Healthy eating projects will be run in year one, physical activity projects will be run in year two and mental well-being projects will be run in the third and final year.
