This project aims to provide members of BAME communities with long-term limiting health issues or disabilities with a suite of support services to address health, education and employability needs in a number of West London Boroughs. Person Centred support will comprise of an initial diagnostic assessment to identify the participants’ needs, barriers and aspirations; a unique individual learning plan, as well as individual career guidance including directed job search support.
