A project to support people with mental health issues and families with children, unable to be discharged from hospital, due to crisis or risk of crisis as a result of hardship. Working with partners the scheme will provide advice and advocacy including access to legal advice, food parcels, and help with accommodation. It will also offer longer term support such as counselling, emotional resilience, primary care liaison, peer mentoring and education, training, employment, and leisure.
