This project will support young people aged 16-29 who are living with multiple complex challenges including mental health issues, offending behaviour, substance misuse, risk taking, self-harm and isolation. Chance 2 Change will, through mentoring, personal goal setting and group work support young people to manage their personal circumstances so that they have the life skills they need to overcome their personal life challenges.
