Passport to Wellbeing (360G-tnlcomfund-0031053432)

This project aims to improve the health and wellbeing of older people, particularly those from the African Caribbean community in Wolverhampton. The funding will help to recruit and train community health and wellbeing ambassadors and create a health and wellbeing toolkit. The ambassadors will work with participants on a one to one basis, finding out about their social care needs, levels of isolation and their eating and exercise patterns. Two group sessions will run each month on various topics.
