The project will use its funding over 3 years to provide specialist support for young adult patients 18 -30 years old their families and friends in their end of life care. The funding will support families during transition care improve the services offered to young patients such as social activities and community outreach through engaging with young people in schools colleges and universities and raising awareness of the support available to those suffering with long-term and critical illnesses.
