Community Triage Hubs (360G-tnlcomfund-0045441403)

The funding will be used to provide early intervention support for people that are homeless or at risk of homelessness across Leicestershire (not Leicester City). This will be delivered by an improved triage service with the creation of “community hubs” across the County enabling face to face support and improved access to the wide range of services provided by The Bridge and its partners for those with complex conditions and circumstances. Through this approach more people will be able to secure and sustain appropriate accommodation and improved quality of life.
