This group will use the funding to continue to deliver a Lanarkshire wide community palliative care service supporting individuals and families affected by life limiting illnesses such as cancer heart disease and progressive neurological conditions. The project develops person-centred action plans to deliver tailored emotional therapeutic and practical support to people and their carers through a hybrid model of remote and face-to-face delivery. The three-year project will support approximately 600 people across North and South Lanarkshire and involve 120 volunteers.
