This group will use the funding to continue its supported referral and signposting service through the continuation of two dedicated staff posts. The group works with people with disabilities long-term health conditions and their carers to identify appropriate services to meet a range of needs and aspirations and provide supported access and follow-up check-ins. This project will work with 10500 disabled people over three years across West Dunbartonshire Glasgow and East Dunbartonshire. 12 volunteers will support delivery of three year project.
