Purpose of Organisation Woking & Sam Beare Hospice and Wellbeing Care, delivers personal palliative care to adult patients with life-limiting illnesses as well as supporting their carers and families. Patients are referred to us by GPs, hospitals and other healthcare providers. Woking & Sam Beare’s mission is to deliver high quality, specialist care and support to residents in Spelthorne, Runnymede, West Elmbridge, Surrey Heath, Woking and North Guildford. We know that one size does not fit all, and our care is most effective when delivered in response to personal circumstance and needs. It is coordinated and in collaboration with a range of healthcare and social care providers, and is accessible to all patients at the time they need us and in their preferred place of care. Through our services, we offer our patients holistic care, which means that as well as managing their medical needs we give practical, emotional, social and spiritual support. Alongside this, we also provide families and carers with guidance and advice so that they remain well and are best able to support their loved one who is affected by illness. We deliver this through the following activities: • Care in the community - enabling people with life-limiting conditions the opportunity to stay in their own home, care home or nursing home. Over 79% of our patients are supported in the community by our team of experienced palliative and end of life care specialists. • 24/7 specialist support on our 20 bed in-patient ward - delivered by palliative care consultants, expert nurses and dedicated health care assistants for patients at end of life or requiring respite care. • Day care in our Bradbury Wellbeing Centre - patients and carers receive out-patient support with wellbeing sessions, complementary therapies, physiotherapy, counselling, clinical care and social activities. • Family support services - patients and their families can access one-to-one counselling (pre and post bereavement) and peer support groups, including specialist groups for children and teenagers. Counselling is provided at the hospice, but most patients facing a life limiting diagnosis are seen in their own homes. NEW as of 07.12.22 Woking & Sam Beare Hospices deliver personal specialist palliative care to adult patients affected by life-limiting and terminal illness resident in six boroughs in Northwest Surrey. It is Woking & Sam Beare Hospices’ mission to deliver high quality specialist care and support to anyone with a life-limiting illness, at the time they need it and in the place of their choice. We know that one size does not fit all, and our care is most effective when delivered in response to personal circumstance and needs, which is coordinated and in collaboration with a range of healthcare and social care providers. We strive to ensure our care is accessible to all patients at the time they need us and in their preferred place of care. The charity cares for over 2,000 patients, their families and carers 24 hours per day, seven days a week, 365 days a year, without any cost to the patient. We help patients to manage their symptoms, which are often complex and why they require the expertise of our Clinical Nurse Specialists and specialist doctors. Through our services we offer our patients holistic care, which means that as well as managing their medical needs we give practical, emotional, social, and spiritual support. Alongside the needs of the patient, we also provide families and carers with guidance and advice so that they remain well and best able to support their loved one who is affected by illness. Our care is delivered in the community, in patient homes and nursing homes, and through our in-patient ward. We also provide care to day patients through our Bradbury Wellbeing Centre, which is located in our hospice and will re-open at the end of June. In addition to our support for patients with a life-limiting illness, we continue to provide pre- and post-bereavement support for children and adults through counselling sessions. Our aim is to help patients have a choice in their care and where it is delivered and to help them achieve the best quality of life until end of life.
