Recipient Org: Description Phyllis Tuckwell is the only Hospice Care service for adult patients, and their families, living with cancer or another terminal illness (such as heart, lung or neurological disease) across the whole of West Surrey and part of North East Hampshire. Our mission is to care compassionately for adults living with an advanced or terminal illness, and those closest to them, so that they have the best possible quality of life and the patients' final days are peaceful...because every day is precious. We also empower our local community, building skills and education in caring for people at the end of their lives, such as local GP's, hospital and care home staff. Last year (1st Apr 2024 - 31st March 2025) - we supported 2,091 patients, across all our services. Also supporting 1,036 carers and family members in addition. We provide a range of free services: - Advice and referral team (referrals to our services) - In-Patient Unit - Hospice inpatient care for patients at end of life, or who need symptom control, and support for families and carers - Living Well - A range of therapies, counselling and information services, delivered in group and outpatient services at Hospice and Beacon Centre, for patients, families and carers. - Hospice at home (where we deliver 80% of our our care, in the community) - Community service for patients living and dying at home (or care home), and support for families and carers. Several services included: 1 - Community ward - Clinical service for patients and families with heightened palliative care needs 2 - Virtual ward - Consultant-led medical and clinical service for patients and families with heightened palliative care needs. 3 - Night support - Advice and skilled Health Care Advisors providing overnight support. 4 - Home care - For stable, slowly deteriorating patients requiring planned support at home. 5 - Home support - Volunteer-led support at home - once a week/fortnight visits to offer patients practical and emotional support. - Education - All our services are underpinned by our education team - to improve our own skills and knowledge, and to enable other professionals across our community to deliver improved palliative and end of life care. Last year (24/25) our education team saw 3,655 attendees across our training calendar and a further 108 healthcare students and visitor placements, a jump from 53 last year.
