Recipient Org: Description | The Hospice team at St Luke's works to provide the highest levels of palliative care. Support extends from diagnosis to end of life care, or survivorship. In addition to patient needs the Hospice adopts a holistic approach to support family, loved ones and carers. St Luke's hospice services include: - Adult & child counselling and support (pre and post bereavement) - In-Patient Unit (8 beds) - 'OneResponse' Line (Operating 24/7 - a central point of communication for patients, their carers’ and family members, as well as health and care professionals coordinating palliative and end of life care in the community). - Day Hospice/Out Patients - Hospice at Home - Carers Support & social work - Specialist Physiotherapy including the South Essex Lymphoedema clinic - Complementary Therapy - Education & Training - Community Palliative Care Team including Dr Melanie Piggott (BM BS, MRCP) – Specialist in Palliative Care for South West Essex. St Luke's is an innovative environment, seeking to advance the provision of health and social care for those of great need in our community. One of only three hospices to deliver Commissioner Requested Services - audited by Monitor - and rated ‘Outstanding’ by the Care Quality Commission (December 2016). |
