Recipient Org: Description At the Hospice we provide CQC rated ‘Outstanding’, specialist palliative and end of life care for those in South West Essex with a life limiting or life changing illness. Patient focus is at the centre of our innovative approach to holistic care which supports patients and families from diagnosis to end of life care, post-bereavement or survivorship. The Hospice team is dedicated to supporting patients and loved ones in their physical, social, emotional and spiritual needs. One of only three Hospices to provide Commissioner Requested Services, we remain committed to growing the services provided to those in the local community. As Accountable Lead Provider for Basildon and Brentwood, and Thurrock Alliance, we work collaboratively with our Health and Social Care partners to deliver efficient and essential care. Care at St. Luke’s Hospice is provided via 19 services delivered in the patients’ primary residence, the Hospice, or virtually using digital technology. These include the Hospice In-Patient Unit (8 bed IPU), Wellbeing Information and Support Hub (WISH), Clinical Outpatients service, Supportive Care Services, Integrated Therapies services, St. Luke’s Hospice Community Services, and our Learning and Development Service.
